This registration form only informs the Governors that a parent is requesting a place here.

Please note that all applications are processed for us by Dudley Local Authority at Westox House, Dudley, using the official Local Authority form.

All parents applying for a place for their child at this school MUST ALSO complete an LEA Coordinated Admission Form.

Please ask for a Local Authority Admission Admission Form at the Office.
